Our website came to the point where it was no longer functional. Therefore, until we find a reasonable, user friendly website format, we shall be operating without a website for the time being.  For access to form, contact Sheri Seguin at vp.sllb.fisp.sls@gmail.com, to be invited to use a google drive with the forms on it or you can E-mail me at msjeannieblake@yahoo.com to have the forms sent to you.
